Care and support at home

The role of the Home Care Team is to provide specialist palliative care to patients in their own home. The Home Care Team consists of Community Macmillan Clinical Nurse Specialists and a Home Support Team. Their work is supported by the multidisciplinary team at Ashgate Hospice and they work closely with the patients own GP and District Nurse.

Home Support Team
Our Home Support Team i provides help to patients and carers in their own homes.

Working with statutory services and other voluntary organisations, our Home Support workers enable patients to be well cared for at home when this is their wish. This is achieved by:
  • providing help to patients and their carers in their own homes
  • providing support and respite for carers
  • helping prevent crisis admission to hospice or hospital
  • assisting discharge home from hospice or hospital
Community Macmillan Clinical Nurse Specialists
The Community Macmillan Clinical Nurse Specialists provide specialist palliative advice and support to patients and their carers. As well as helping patients to achieve the best possible quality of life, they also provide education and act as a resource to staff in the community.
